What happened here?

I just pulled a 4:11 punkin for my friend's 39 from a 34 rear end that had been converted to use as a trailer. One axle pulled normally, but the other one dropped this 1/16 thick 'c' spacer out. Then the bearing wouldn't come out because the outer race was split and hanging up on the backer plate. I had to grind off the rivets and remove the plate to get it out.
The bore measures about .018 over the nominal 2.750, which the other side measures.
Who has a theory?
