What does the "follow" column mean and how do you use it?

Rob,
Just found what you are referring to, one of four information blocks (Votes, Follows, Views, Comments)
Essentially, you can click-on the 'Follow' to subscribe to the thread, which means you should get a notice if there are any updates to that thread (email or 'pop-up', depending on your preferences).

On the right hand side of the top level discussion as shown on the discussions page it appears there. You have "Votes Follows Views Comments" in that sidebar.
If you click follow it creates a bookmarked discussion for you which you will see appear over the right hand side of the web page
If you click Votes then that will up the votes count ; you can use that as filter in how you display threads1 -
